APPLICATION All persons interested in participating in an ALMS-Missions Trip, must fill-out an Application and submit such unto ALMS by Web, Mail or Email at least 90-days in advance of our Trip-Departure. All such Applications must be accompanied by a Copy of each Item requested below. All Applications are Free, but please be aware that a $500-Deposit, is due 100-Days prior to our Trip-Date. ALMS-Applications may be Printed or Filled-out on-line.
PASSPORT All persons interested in participating in an ALMS-Missions Trip, must have a Passport. If any person does not have a Passport, one may Apply for one at any US-Post Office or may Fill-Out a Passport App on-line. For First-Time Passport Applicants, click: FIRST. For those whose Passport was Lost or Stolen, click: LOST . Those who need to Renew their Passport, please click: RENEW . Applying for Passport, takes up to 8-weeks. You can Order an Expedited Passport online, but costs more and takes 2-3 weeks. However, those who need to obtain a Passport within 1-2 weeks, immediately go to the nearest Passport Agency and Apply in person.
PERMISSION All persons from 13-17 years of age, must obtain Written Permission from at least 1-Parent or Legal Guardian, to participate in an ALMS-Missions Trip. All other Persons 12-years of age and under, must be accompanied by a Parent/Legal Guardian or an Adult Relative that is at least 21-years of Age and Approved by ALMS to be eligible to participate. ALMS-Permission Slips, may be Printed or Filled out on-line, ONLY by a Parent or a Legal Guardian.
PHYSICAL All persons interested in participating in an ALMS-Missions Trip must obtain a Physical by a Doctor of your own choosing. Written proof of such Physical must be submitted with Application as verification of the same. All persons must be deemed Physically and Mentally fit and capable of enduring Missions Trip. All persons on Meds must bring/take Prescribed Meds accordingly. Failure to obtain a Physical within set Time-Frame will result in denial of Application. Shots are not required by Law.
